Patriot Platinum is an A- rated comprehensive Travel Medical insurance plan that provides medical coverage and other covered travel and emergency services for visitors to the U.S during their travel and temporary stay outside of their home country. Coverage begins once the policyholder has left their home country and is in transit to or has entered one of the eligible destination countries. The coverage duration is available from 5 days to 365 days. While in the U.S, after the deductible is satisfied within the United Healthcare PPO network, the plan will pay 100% of eligible medical expenses up to the policy maximum. | Q2: Are pre-existing conditions covered by the Patriot Platinum plan? Pre-existing conditions are not covered by the Patriot Platinum plan. However, the Patriot Platinum plan covers for an acute onset of a pre-existing condition. It covers, with varying limits by age, up to $1,000,000 for people up to 69 years of age; For age 70 and over, the plan does not provide coverage for acute onset of a pre-existing condition. It is up to the attending physician to determine if a particular medical situation is related to pre-existing condition or not, and also if it would be considered as an acute onset or not. | Q3: What is an acute onset of a pre-existing condition? An acute onset is a sudden and unexpected outbreak or recurrence of a Pre-existing Condition, which occurs spontaneously and without advance warning either in the form of Physician recommendations or symptoms, and is of short duration. It has to be an acute medical condition that requires immediate medical attention. More conditions and restrictions apply. The cost of Patriot Platinum varies based on several qualifying criteria, such as the traveler's age, duration or length of trip, selected coverage benefit limits, and the selection of additional riders. Typically, it can cost about $9 a day for a person 50 years of age, when selecting coverage area or destination of travel as the USA. |

| Q9: Why are there state restrictions and will it impact me if I visit one of those states? Each insurance company may have their own state-specific regulations on where plans can be sold. These state restrictions only impact residents from those specific states from purchasing these plans. However, if you are traveling to the United States, you will be covered in all 50 states during your active policy period. |
| Q10: Can I extend the plan, if I plan to stay longer? Yes, you can extend the policy. You would need to extend the plan at least two days before the policy expires. There would be an extra $5 extension charge. |
| Q11: Can I cancel the plan, if I plan to go back earlier? Yes, you can cancel and get a refund for the Patriot Platinum Plan, if you need to go back earlier than plan. The following rules apply:

| Q12: Can I cancel only one person from the policy? You cannot remove one person from the policy if other travelers are included. We recommend letting the current policy lapse with all travelers or canceling and repurchasing for the remaining travelers as per the policy terms. |

| Q14: When will I receive my ID card? You can print out your ID card from your confirmation e-mail and it will be used as proof of insurance. While filling out the online application, you will receive confirmation in your e-mail. If you select paper mail/snail mail then you will receive the ID card in the mail within 5-10 business days. | Q15: What if I am traveling to multiple countries, will I be covered? Coverage will be provided in all eligible countries visited with the exception of your Home Country and any restricted countries listed in the policy documentation. Coverage will only be offered within the policy start and end dates. Please note: If any part of your trip includes the United States, please select it as your destination country. |

Patriot Platinum can cover for COVID-19 and will be treated the same as any other eligible medical condition so long as the virus is contracted after the policy effective date and you have departed from the country of residence. Based on the desired coverage length you have selected, you have to make the full payment at once. Since this plan is extendable you can extend it anytime you would like to before the policy expires. Only the primary destination in the application will be listed on the policy documents, but rest assured that all other eligible inputted countries will be covered during the active policy period. |
| Q20: When does my coverage start and end on my travel medical insurance plan? Coverage on a travel medical insurance plan will begin on the date of departure and once the traveler has left their home country. Coverage areas are anywhere outside one's home country as long as the policy is active. The coverage will end immediately once the traveler has entered back into their home country. |
| Q21: Will my Patriot Platinum plan be extended /renewed automatically? No, your Patriot Platinum plan won't be extended or renewed automatically. You have to sign into your account online and activate the extension or renewal yourself. Not all plans are extendable or renewable, so be sure to review your policy terms and conditions for eligibility. |
| Q22: Does the Patriot Platinum plan cover for the coronavirus vaccine/boosters? The Patriot Platinum plan does not cover for the COVID-19 vaccine/booster, but will cover COVID-19 treatment and services the same as any other eligible condition if contracted after the policy effective date. If your school or educational program does not any have specific health insurance requirements to meet, this plan can offer short-term health insurance coverage while you are abroad. It is important to confirm with the school or educational program to ensure that any short-term health insurance plan will be accepted by them. |
| Q26: What's the Rx Discount Card that comes with this plan? This is a discount savings program available to every certificate holder of the Patriot travel plans. This program allows card members to purchase prescriptions at one of over 35,000 participating pharmacies in the U.S. and receive the lower of 1) Universal Rx contract price or 2) the pharmacy regular retail price. This discount program is not insurance coverage. It is purely a discount program to purchasers of the Patriot travel plans. | Q27: Why do corrections take 3-5 business days and can't be a quick fix? The insurance provider recommends that you submit any correction requests in writing as your policy is a legally enforceable document. Following receipt of the correction, we send the request to the insurance provider; normally, the updates require 3-5 business days. They are processed in the order that they are received. We advise double-checking the information on the application before completing the purchase to prevent any errors and corrections. |

The Adventure Sports Rider offered in the Patriot Platinum plan is available for travelers up to the age of 65 and must be purchased at the time of application. The rider is designed to cover a policyholder while participating in the following activities if the activity is solely for leisure, recreation or entertainment purposes:abseiling, BMX; bobsledding; bungee jumping; canyoning; caving; hot air ballooning; jungle zip lining; parachuting; paragliding; parascending; rappelling; skydiving; spelunking; wildlife safaris; and windsurfing. |

The Device/Cellular Protection rider is an optional coverage that certain residents can purchase to cover their cellular phone in case of damages during travel. The optional rider can either provide coverage for repair costs or some coverage for replacing the cellular phone. |

The Accidental Death and Dismemberment rider is a choice exclusively offered to the main insured person, and it must be acquired at the time of application, with a minimum requirement of 3 months of coverage. | Q33: Why is my premium $10 when the daily rate calculated is less? To purchase the Patriot Platinum policy, the minimum premium must be at least $10.
